Collaborative Project Weekly Self-Assessment Form

Description (for Instructors)

This activity, in which students describe and evaluate their contributions to group work, helps group members hold themselves and one another accountable during extended collaborative activities, such as the Collaborative Analysis Assignment. This prompt-guided assessment worksheet also teaches students how to perform critical self-reflection—a skill that’s integral to both the Performance Review Assignment and professional employment.

Students can submit their Weekly Self-Assessment Forms either as a print copy or by using a submission tool on the course management system of the instructor’s choosing (i.e., Moodle, Canvas, etc.).

Materials

Explanation (for Students)

1. Fill out Collaborative Project Weekly Self-Assessment Form:

Every week during collaborative work, you must fill out and submit the following document: Collaborative Project Weekly Self-Assessment Form.docx.

In this form, you will complete the following questions with as much specificity and truthfulness as possible:

1. On a scale of 1-10, how much did you contribute to the project this week?

_______ (1 = Not at all, 10 =  I give it my all)

2. Did you complete the tasks assigned to you by the manager and group (Mark ‘X’ next to the best answer)?
